Your Hospital or Doctors Might Leave Your Plan's Network in 2027: What Kissimmee Members Can Check Before Dec 7

Updated: 1 hour ago
You may have heard that a hospital or some doctors could be out of your plan's network on January 1, 2027.
Nothing changes before the contract ends, and contract talks sometimes end in a new agreement. But your decision windows are open now. Here's what to check by type of coverage, and the dates that matter for neighbors in Kissimmee, St. Cloud, Poinciana and across Osceola County.
What happens if my hospital or doctor leaves my plan's network?
Until the contract ends, nothing changes, so keep your appointments. If the provider goes out of network, your plan may cover less or nothing for non-emergency care there, depending on plan type, and you'd normally choose an in-network provider.
An HMO usually covers only in-network care, except emergencies and some urgent care. A PPO may still pay something out of network.
Network changes can happen any time
According to Medicare.gov, Medicare Advantage plans can add or remove providers during the year, and providers can leave, too. Your plan must still protect you from interruptions in medically necessary care.
What notice you should get
Federal rules (42 CFR 422.111(e)) require Medicare Advantage plans to mail notice before a contracted provider leaves: at least 45 days ahead for primary care and behavioral health providers, plus one attempt by phone, and at least 30 days ahead for other providers. Keep that letter.
Contract talks can end in a deal
Agreements are sometimes signed close to the deadline, so check again in late December.
Which coverage do you have?
Find your row, then read the matching section.
Coverage type | Main window to change | Who to call |
Medicare Advantage (including HMO and PPO) | Oct 15 – Dec 7, 2026 for Jan 1; then one change Jan 1 – Mar 31, 2027 | Number on your card; 1-800-MEDICARE |
D-SNP / Medicare and Medicaid | Oct 15 – Dec 7, 2026, plus monthly options (see below) | Number on your card; 1-800-MEDICARE |
Marketplace (HealthCare.gov) | Nov 1, 2026 – Jan 15, 2027; choose by Dec 15 for Jan 1 | Number on your card; HealthCare.gov |
Employer or group plan | Your employer's open enrollment | HR or benefits office |
Florida Medicaid plan only | Medicaid has its own rules | Number on your Medicaid plan card |
Have Medicare and not sure your doctors or hospital are in your 2027 network? Bring your list to a no-cost Medicare review. There's no cost to you for the review and no obligation to enroll.
If you have Medicare Advantage or a D-SNP
Use Open Enrollment, October 15 to December 7, 2026, to check whether your doctors and hospital are in the 2027 network of your plan or another plan. Switch by December 7 for coverage that starts January 1.
Steps before Dec 7
List your doctors, specialists, hospital, pharmacy and drugs. Look each one up for 2027 (see the checklist below), and compare plans available in Osceola County.
Is there a special enrollment period for a network change?
Only if Medicare decides the change is "significant," case by case. It isn't automatic; a provider leaving doesn't by itself give you one.
If CMS grants one, your plan must send you a notice. Per Medicare.gov and CMS guidance, it may apply if you're assigned to, getting care from, or got care in the past 3 months from the provider that's leaving. It starts the month you're notified, ends 2 calendar months later, and can be used once to switch Medicare Advantage plans or go to Original Medicare with a drug plan.
Don't wait for one; use December 7 if you can. If a network change puts your care at risk, call 1-800-MEDICARE and ask whether you qualify.
Medigap: if CMS calls the change significant, the plan's notice is supposed to explain any Medigap guaranteed-issue rights. Otherwise, Florida insurers can usually ask health questions.
After Jan 1
From January 1 to March 31, 2027, Medicare Advantage members can make one switch to another Medicare Advantage plan or return to Original Medicare with a drug plan. Coverage starts the first of the next month.
If you have Medicare and Medicaid or Extra Help
Per Medicare.gov, with Medicaid or Extra Help you can change once a month into Original Medicare with a drug plan, or switch drug plans. With full Medicaid, you can join or switch to an integrated D-SNP once a month where available. Changes start the first of the next month. These options aren't available to people flagged "at-risk" by a drug management program. Before changing, ask how it affects your Medicaid plan.
Retiree plans through a former employer
Check with your former employer or union before dropping anything. Leaving may affect your retiree coverage.
If you have a Marketplace (ACA) plan
Open enrollment on HealthCare.gov runs November 1, 2026, to January 15, 2027. Choose by December 15 for coverage starting January 1. Plans picked December 16 to January 15 start February 1.
Before your plan renews, check each 2027 plan's directory for your doctors and hospital. See HealthCare.gov's dates and deadlines and, for changes outside open enrollment, its special enrollment period page.
If you have coverage through work
During your employer's open enrollment, ask HR which plan options include your doctors and hospital for 2027, and whether the plan has out-of-network coverage. Employer open enrollment dates vary.
What is a continuity of care request?
If you're mid-treatment when your doctor or hospital leaves the network, you can ask your plan to let you keep seeing them at in-network cost for a limited time. Call the number on your card and ask for it by name.
Employer, Marketplace and other individual plans: under the federal No Surprises Act, you may qualify if you have a serious and complex condition, inpatient or institutional care, scheduled non-elective surgery and post-op care, pregnancy, or a terminal illness. Per CMS, it can last up to 90 days from the plan's notice, or until you no longer qualify, whichever comes first.
Florida HMOs: Florida law adds protections for some HMO members; self-funded employer plans may not be subject to it.
Medicare Advantage: plans must protect you from interruptions in medically necessary care. Ask your plan about transition or continuity options, and call 1-800-MEDICARE if your care is at risk.
When you call, have your treating doctor's name, your condition and treatment dates, upcoming surgeries or appointments, and the notice letter. Ask the plan to confirm any approval in writing.
Will emergency care still be covered?
Yes. In an emergency, go to the nearest ER.
Medicare Advantage plans must cover emergency care anywhere in the U.S., in or out of network, with no referral. With Marketplace or employer coverage, call the number on your card to ask how out-of-network emergency care is handled. Follow-up care after you're stable may be handled differently, so ask your plan.
How do I check if my doctor or hospital is in network for 2027?
Follow these six steps for each doctor and location.
Make your list. Write down every doctor, specialist, hospital, lab, imaging center and pharmacy you use, with exact locations.
Search the official tool. Medicare: at Medicare.gov/plan-compare, enter your ZIP and search your doctors and facilities to see which 2027 Medicare Advantage plans include them. Marketplace: use plan directories on HealthCare.gov. Employer: use HR's directory.
Check the plan's own directory. Make sure you're looking at 2027, not 2026.
Call member services. Use the number on the back of your card. Ask: "Is [doctor and location] in network for 2027? What would I pay out of network? Is there a continuity of care option for me?" Write down the date, rep's name and reference number.
Call the doctor's office. Ask which 2027 plans they'll take at that location, and whether they're taking new patients.
Check again in late December. Contracts can be signed, or end, at the last minute.
If a new agreement is signed
Nothing changes for that provider, but the checklist still applies. Networks change every year, so confirm your doctors for 2027 anyway.
What to bring to a review
Bring your Medicare card and plan card, any network or termination letter, your doctors and their locations, the hospital you prefer, your drug list with doses, your pharmacy, and dates of upcoming treatment. We can meet in person in Kissimmee, by phone or by video.
There's no cost to you for the review and no obligation to enroll. Learn what a no-cost Medicare review covers or see Medicare options in Kissimmee.
Frequently asked questions
What happens if my hospital or doctor leaves my plan's network?
Until the contract ends, nothing changes. After that, your plan may cover less or nothing for non-emergency care there. Check your plan type, ask about continuity of care, and compare 2027 plans before your enrollment deadline.
Can I change my Medicare plan if my doctor is leaving the network?
Yes, during Open Enrollment, October 15 to December 7, for coverage starting January 1. If you're in Medicare Advantage, you can also make one change January 1 to March 31.
Is there a special enrollment period when a provider leaves a Medicare Advantage network?
Only if Medicare decides the network change is significant. It's decided case by case. If you qualify, your plan will notify you, and you'll have that month plus two more months to switch. Don't count on it. Use December 7 if you can.
What is a continuity of care request?
It's a request to keep seeing a doctor or hospital that's leaving your network, at in-network cost, for a limited time while you finish treatment. Pregnancy, scheduled surgery and serious ongoing treatment often qualify. Call the number on your card.
Will my plan still cover emergency care at an out-of-network hospital?
Yes. In an emergency, go to the nearest ER. Medicare Advantage plans must cover emergency care in or out of network. If you have other coverage, call the number on your card to ask how out-of-network emergency care is handled.
How do I check if my doctor is in network for 2027?
Use Medicare.gov/plan-compare (Medicare), HealthCare.gov (Marketplace) or your employer's directory. Then call the number on your card and the doctor's office to confirm the exact location for 2027.
What if I have Marketplace or employer coverage?
Marketplace: compare plans November 1 to January 15; choose by December 15 for January 1. Employer: ask HR during your open enrollment which options include your doctors and hospital.
